
Community-Based Services
Youth Homes Community-Based Services provide mental and behavioral health support throughout Contra Costa County for youth and young adults ages 3 to 26. These services include Individual, Family, and Group Therapy, Intensive Care Coordination, Therapeutic Behavioral Support, and Life Skills Coaching.

Individual Therapy provides a safe, one-on-one space where youth develop coping skills and work through trauma or personal challenges with a therapist.
Family Therapy focuses on strengthening relationships, improving communication, and resolving conflicts by involving caregivers and other family members in the healing process.
Group Therapy allows youth to connect with others facing similar challenges, build social skills, and learn healthy ways to manage emotions.

Intensive Care Coordination (ICC) is a collaborative approach designed to support youth by coordinating services across multiple systems. ICC focuses on developing and implementing individualized care plans that bring together the youth, family, mental health providers, school staff, and other key support systems to ensure a comprehensive approach to treatment. A dedicated care coordinator works closely with the family to identify hopes and visions for the client, and facilitate collaboration among service providers, ensuring that all aspects of the youth’s well-being are considered. ICC helps families navigate resources, improve stability, and achieve sustainable positive outcomes.

Therapeutic Behavior Services is a short-term, intensive service designed to help families manage their youth's challenging behaviors that may put them at risk of hospitalization or residential placement. In collaboration with families, mental health providers, and other support systems, TBS Specialists develop customized treatment plans that focus on strengths and set measurable goals. Through skill-building and therapeutic strategies, TBS Specialist empower families to create lasting, positive change even beyond the duration of services.

Life Skills Coaches at Youth Homes support transition-age youth and young adults as they develop the practical skills they need to navigate daily life and build a successful future. Through individualized coaching and hands-on experiences, clients learn essential skills such as financial literacy, time management, cooking, communication, and job readiness through our Employment Development Program. By practicing these critical skills, Youth Homes ensures that clients are better prepared for adulthood as they transition from the foster care system to independent living.
