Our Challenge

As under-resourced and vulnerable Transition Age Youth (TAY) (ages 17-21) shift into independent living, they must deal with the unexpected realities of becoming an adult. Poverty, homelessness, unemployment, substance abuse, incarceration, and hospitalization are all real risks for the individuals who qualify for TAY services and these critical programs can make the difference between crafting a life filled with meaning and purpose or falling through the cracks of society.

Our Approach

Youth Homes’ Transition Age Youth Aftercare and Transition-Age Youth-Full Service Partnership (TAY-FSP) focus on providing clients with mental and behavioral health support, with TAY-FSP focusing specifically on young adults (aged 16-25) living with severe emotional challenges and life stressors.

Overlapping services within Youth Homes, as well as other community services, help these youth build a network of support that can aid them through the transition period. Building wellness, security, and independent living skills, reducing homelessness, gaining employment, and encouraging ongoing academic pursuits, are all goals of these services. Young adults are assigned a team based on their individual needs, which may include a primary therapist, youth advocate (former foster youth), substance abuse counselor, and/or an independent living skills coach, to help formulate and realize achievable goals for their future.

What to expect

Employment Development Program (EDP)

Youth receiving services from Aftercare and TAY-FSP also have the opportunity to participate in our Employee Acceleration Program. In partnership with the Department of Rehabilitation, youth are able to earn a wage and gain workplace skills in an environment that is safe and supportive. They receive coaching, feedback, and support via the Youth Homes Life Skills team.
